HOW TO COMPLETE THE APPLICATION FORM

1. Study the Unishire EZ Credit Application Form.
2. Fill in all the spaces. All items are required, unless otherwise specified.
3. Important: Do not go to other pages while you are filling up the form. All entries will be lost.
4. After you have completed the form and reviewed/edited your entries, please print your application form. Sign on the spaces provided and submit to us, through Fax, Mail or in person at UNISHIRE or nearest Holy Gardens office, along with the photocopies of the required documents.
5. Together with the completed Unishire EZ Credit Application Form, kindly submit the following required documents:

   Basic Documentary Requirements (for all loan amounts)

   1. 2 pcs 2x2 latest picture
   2. Latest Community Tax Certificate (cedula)
   3. Proof of billing (Meralco, Telephone, Water, Credit Card, Bank Statement, etc.)
   4. Photocopy of both sides of two (2) valid identification card with signature*
   5. Photocopy and Original copy of any of the following:
   1. Certificate of Ownership (CO) and Deed of Sale (DOS)
   2. Certificate of Entitlement (CE) and Offer to Purchase (OTP)
   3. Transfer Certificate of Title (TCT) and Deed of Absolute Sale (DOAS) of any Unisun Homes Subdivision

   (for loan amounts in excess of P51,000)
   For employed applicants

   1. Any of the following income documents:
      1. Proof of income (latest payslip, bank remittances, etc.)
      2. Photocopy of latest credit card statement and/or credit card number
   2. Photocopy of all pages of latest Income Tax Return (ITR) or W-2 duly stamped by BIR
   
   For self-employed applicants
   1. Any of the following documents:
      1. Photocopy of latest audited Financial Statement with BIR or bank stamp
      2. Photocopy of latest credit card statement and/or credit card number
   2. Photocopy of all pages of latest Income Tax Return (ITR) or W-2 duly stamped by BIR
