Documentary Stamp Tax: P1.50 for every P200.00 of the Approved Loan Amount

determined in accordance with the Terms and

Conditions governing the Unishire EZ Credit

Late Payment Fee: 4% of unpaid installation due (cummulative)

Warrants that all information given in this Application Form are true and correct and authorize Unishire to investigate the information from whatever sources Unishire may consider appropriate.
Agree to submit a photcopy of all pages of my latest Income Tax Return (ITR) every year for the entire loan period of my Unishire EZ Credit
Signify my agreement to the Terms and Conditions governing the Unishire EZ Credit and to the other Loan Documents relative to my Unishire EZ Credit, which Loan Documents are incorporated herein by reference, and confirm that I have read and understood the same and agree to any amendments or supplements hereto. Capitalized terms used herein and defined in Terms and Conditions governing Unishire EZ Credit shall have the meaning ascribed to them herein.
Understand that Unishire may, at its sole discretion, deny my Loan Application with no obligation on its part to advise of such denial or furnish the reason hereof.
Understand that, in the event that Unishire agree to grant me a Unizhire EZ Credit, nothing herein shall be construed as obligating Unishire to grant me the Unishire EZ Credit in the Loan amount stated by me in Box2 and for the Loan Period stated by me in Box#3 and the final Approved Loan Amount and Loan Period of my EZ Credit shall be that agreed upon between me and Unishre. In view thereof, I hereby authorize Unishire to fill in the final Approved Loan Amount and Loan Period agreed upon me and Unishire in the spaces provided therefore herein below.
Agree that from the proceeds of my Unishire EZ Credit will be deducted the documentary stamp taxes, and processing fee.
Agree to be included in the customer database of Unishire, and opt in to receive messages on special deals and promotions. Should I decide to opt out, I will have to inform Unishire Hathaway Financing Company in writing of my request.
